Without fail the assigned team arrives at our home on time. They are friendly, properly dressed and groomed. Most importantly the team works diligently to perform all proper and agreed upon cleaning services within 90 minutes of arrival.

The maids are always very kind to our pet family while they are here. It must be hard for them. Our pet family does not always respect the cleaning that the merry maids work so hard to accomplish!

The vertical space between "Not" and "likely"; and the vertical space between "Very" and "likely" means to a quick reader that the first line of symbols relates to negative and the second line relates to positive!
When I examined the content more carefully, I realized what you mean. But you need to design your questionnaires to be understood by good reades, poor readers, quick readers, thughtful readers, etc.
My answers to the questionnaie would be very favorable, but I am wutholding them in the hope that you will refer my comments to someone who has some authority over design of your questionnairs.
I don't want my time, in typing this note, to have been wasted.

Perhaps my skill on the compur\ter is inadequate, but I can't find any way to "erase" my mark in a circle in case I reconsidered my response and change my response. This something else I want you to refer to some one in higher authority.
