i never cancelled my membership. I don't know why I received this survey.

I didn't cancel my membership. I pay annually each year and the gym has been closed so I've not been back to renew.

Wasn't aware my membership had been canceled. I wasn't given any communication reminding me to renew.

I never felt uncomfortable, I would of kept working out at the gym during this whole pandemic.
COVID - not being told when closing or reopening and continuing to be charged monthly membership without notification. Was OK until you charged yearly membership with no access to club and no notification.
Will not return until I feel safe and only then if fees do not go up.
